I do hereby agree to indemnify and hold harmless United Through Reading, its employees, volunteers or agents from any and all claims or causes of action that may arise out of performance of my assigned duties as a volunteer. I waive any right of action I have against United Through Reading in consideration of my participation as a volunteer for United Through Reading.

During the course of volunteering with United Through Reading, I will be exposed to concepts, ideas, methods, techniques, materials and data that have been developed by and/or presented in a certain format by United Through Reading. As a condition of volunteering, I understand that I am required not to use the information outside of the scope of my volunteer duties with the Foundation.

I hereby authorize the United Through Reading, a California nonprofit corporation, to take photographs and videos depicting me either alone or in a group. Moreover, I hereby give said United Through Reading the right to use, reproduce, adapt, display, and/or distribute copies of the photographs and videos for any purpose whatsoever related to the business and educational activities of the United Through Reading and in all media of expression.

I understand that making any false statement on this application will be sufficient for discharge. I hereby guarantee the correctness of the above statements. I hereby authorize you to contact my references. I understand that this is an application only and not a guarantee of a position.
